First Visit (Evaluation)

Dr. Cook will examine the patient, answer any questions, and evaluate the need for further study. There is no charge for this visit.

Second Visit (Records)

Photographs, x-rays, and impressions for models are taken. These diagnostic records are studied and a specific treatment plan is determined.

Third Visit (Consultation)

Diagnosis and a recommended treatment plan including estimated treatment time will be discussed in detail. Financial arrangements will be set up at this time to extend your payments over a period of months following an appropriate down payment.

If full payment is made at the onset of treatment, we will offer a discount. Advance payment saves us time, expense in billing and carrying costs.
